Understand
IOTN has dental health and aesthetic components that describe treatment need; it is not a substitute for clinical judgement or a guarantee of NHS eligibility.
Assess
Record the relevant occlusal feature accurately and use current local commissioning criteria. Consider the patient's concerns and ability to maintain orthodontic appliances.
Apply
Explain what the grade means and does not mean. Referral should state the clinical concern and urgency, especially for trauma risk, ectopic eruption or functional displacement.
Key distinctions
IOTN grades treatment need, not expected outcome or consent. The aesthetic component reflects appearance and the dental health component captures features such as overjet or crossbite according to defined criteria. Funding rules and local pathways can change. A referral should communicate the measured problem, its consequences and the patient's wishes alongside any grade.
Think through a case
A referral gives an IOTN grade but no description of the feature producing it. The specialist cannot judge urgency or patient concern from the number alone.
The distinction to remember
Do not equate eligibility criteria with automatic clinical benefit or a patient's wish for treatment.
